Self-Directed Supports

YOUR Life. YOUR Way.
Making Choices. Becoming Empowered.

What is Self-Directed Supports?
Self-directing your services offers you choice as it relates to your outcomes, desires, wants, or needs.
You choose:
- Who provides your services
- When services are provided
- Where your services are provided
- How your services are provided
“SDS provides me with the ability to take care of my needs in my safe environment.” – Lakeland Care Member
“I like having the freedom to choose who cares for me.” – Lakeland Care Member
If you choose to self-direct, you have the option to direct one or more of your services. It is not an “all or none” approach.
Why choose to Self-Direct Your Services?
- SDS offers you choice.
- SDS offers you flexibility.
- SDS gives you a voice.
- SDS increases your independence.
