Eligibility and Enrollment for Lakeland Care’s Family Care Program

Lakeland Care’s Family Care Program is intended for Wisconsin residents who are adults with physical, developmental, or intellectual disabilities, or seniors aged 65 or older. This page serves as a resource for eligibility and provides information on how to become a member.

Eligibility:

To be eligible for Family Care, you must:

  • Be an adult (age 18 or older) with a physical disability, developmental or intellectual disability, or a frail elder, age 65 or older
  • Be a resident of one of our service areas (see below for the list of our service areas)
  • Be financially eligible for Medicaid
  • Be functionally eligible as determined by the Wisconsin Adult Long Term Care Functional Screen

Only individuals that are residents of one of the counties in our service area can enroll in Lakeland Care. Our service area includes these counties in Wisconsin: Brown, Calumet, Door, Florence, Fond du Lac, Forest, Kewaunee, Langlade, Lincoln, Manitowoc, Marathon, Marinette, Menominee, Oconto, Oneida, Outagamie, Portage, Shawano, Vilas, Waupaca, Winnebago, and Wood.

How to Become a Member of Lakeland Care:

Information and assistance are readily accessible through your county’s Aging and Disability Resource Center (ADRC). The ADRC serves as a valuable resource, offering personalized consultations to discuss your specific needs, available options to address them, and the eligibility criteria for enrolling in public programs such as Lakeland Care’s Family Care Program.

Should you qualify, you will have the opportunity to voluntarily enroll in the program of your preference, with the flexibility to enroll or re-enroll at your convenience. The ADRC stands ready to guide you through the enrollment process and provide any necessary support. Lakeland Care offers the Family Care Program under a contract with the Wisconsin Department of Health Services. Lakeland Care is proud to meet the high standards required for certification by the State as a managed care organization.

Staying Enrolled:

To stay enrolled as a Lakeland Care member, you must:

  • Remain a resident in our service area
  • Continue to meet financial and functional eligibility requirements
