Our Family Care program is designed to help eligible elders and adults with disabilities live independently – at home and in their community. Members partner with our care teams and local providers to obtain the support and services they need.

Our Locations
Lakeland Care is now serving the following counties in South Central Wisconsin: Adams, Waushara, Marquette, Green Lake, Columbia, Dodge, Dane, Jefferson, and Rock. Lakeland Care also offers the Family Care Program in the following counties: Brown, Calumet, Door, Florence, Fond du Lac, Forest, Kewaunee, Langlade, Lincoln, Manitowoc, Marathon, Marinette, Menominee, Oconto, Oneida, Outagamie, Portage, Shawano, Vilas, Waupaca, Winnebago, and Wood.
